Studio Visitor Waiver

I certify that I am the parent/caregiver/guardian of the "Visitor" and agree to the following in relationship to the services that Pinta & Play LLC provides, at the Pinta & Play Bilingual Studio, located at 209 6th Street Suite 100Hoboken, NJ 07030, and any other facilities used by Pinta & Play®, for classes, events, or activities (hereafter, collectively referred to as "The Facilities"). I also certify that the child/all visitors with me are up to date on all of their shots/immunizations.

RELEASE AND WAIVER OF LIABILITY. Visitor recognizes that there are hazards and risks connected with open play, birthday parties and scheduled Pinta & Play classes. As such, the Visitor hereby exempts, releases and holds harmless Pinta & Play LLC and its owners, employees, subcontractors and volunteers from any and all claims or causes of action arising out of personal injuries suffered by themselves/his/her child during the course of play and/or classes. It is further acknowledged by the Visitor that the Visitor is responsible for the safety of the child they have brought to The Facilities. The Visitor further agrees to defend, indemnify and hold harmless Pinta & Play LLC against any loss, damage or expense incurred by reason of any claim or liability based upon personal injury or property damage/loss arising out of the action of Visitor or his/her child.

MEDICAL RELEASE: Visitor understands that Pinta & Play LLC is taking full precaution to keep The Facilities clean and sanitized to avoid the spread of viruses and other diseases. Visitor hereby exempts, releases and holds harmless Pinta & Play LLC and its owners, employees and volunteers from any and all claims or causes of action arising out of sickness due to any health reason suffered by themselves/his/her child.

Visitor also confirms to the best of his/her knowledge that neither the Visitor nor the child has been sick or showed any symptoms issued by Center of Disease Control (CDC) 5 days prior of the visit to The Facilities. Please do not visit The Facilities if the Visitor or child has had a fever in the last 24 hours. Visitor further acknowledges that Pinta & Play Bilingual Studio is not a "drop-off" facility for open play, and parties and that an authorized adult over the age of 18 must be present and/or responsible for the child at all times.

GENERAL RULES AND REGULATIONS. Visitor acknowledges that Pinta & Play LLC operates under rules and regulations established for the safety and protection of its visitors, and agrees to be bound by such rules and Regulations as posted in the "Terms and Conditions". Hours, service, regulations and policies are subject to change from time to time, without prior notice, in the sole discretion of Pinta & Play LLC. Visitor agrees to accept such reasonable change(s) as a condition of Visitorship. As a Visitor, I understand that I am entitled to use the Pinta & Play Bilingual Studio space within the scope of the Visitorship that I have selected. Signature by signing this document I am agreeing to the terms outlined in the above agreement. I certify that my electronic signature is as valid as my physical signature.

TERM. this waiver covers all classes, programs, and activities at The Facilities during the enrollment period and any future enrollment period. Waivers may be re-issued and required on a periodic basis.
